Proceed in 3 easy steps:

1. Make an appointment at the clinic of your choice. Click here to see the list of clinics.

2. Print the attached pdf file and fill it out.

3. Fax the document to 514-624-5044 or scan it and email it to info@actionsportphysio.com. If you prefer, you can also bring it with you at your first visit.
